RESUMO

This article describes a clinical case of bladder necrosis developed after X-ray endovascular embolization of prostatic arteries of a 62-year-old patient with a verified diagnosis of BPH (benign prostatic hyperplasia). The complication resulted in the necessity of urgent surgical intervention, namely, laparotomy, cystprostatectomy and bilateral percutaneous nephrostomy. In the early postoperative period the patient had intense cutting pain in the left side of the abdomen. Examination revealed the inflow of small intestinal contents through the pelvic drainage, which was the reason for relaparotomy, abdominal cavity revision, uturing the small intestine perforation, suturing the small intestine pre-perforation, sanation and drainage of the abdominal cavity in an emergency procedure. The patient was discharged in a satisfactory condition under the supervision of a urologist by m/w on the 36th day after endovascular embolization of prostatic arteries. The eight months after discharge, the patient underwent a successful Brickers operation on creating an alternative urinary diversion route at the First Sechenov Moscow State Medical University of the Russian Federation.

RESUMO

A clinical case of surgical treatment of a patient with autosomal dominant type of polycystic kidney disease, stage 5 of chronic kidney disease and secondary arterial hypertension is presented in the article. The technique of single-stage bilateral laparoscopic nephrectomy, patented by the authors, is described. The practicability and safety of a simultaneous bilateral procedures was demonstrated, as well as the advantage of laparoscopic access for this type of surgical interventions. The successful and prompt procedure allowed the patient to undergone to allotransplantation of a cadaveric kidney as soon as possible.

For the purpose of improving diagnosis of prostate cancer (PC), the authors used an innovative HistoScanning device. According to the standard procedure accepted at the clinic, 326 patients admitted with suspected prostate cancer were evaluated. Patients with elevated prostate-specific antigen (PSA) were divided into three groups depending on the degree of its increase, which ranged from 1.09 to 209 ng/ml; Group 4 (conventional control) consisted of patients with BPH, and Group 5 (control) consisted of healthy men aged 21 to 28 years. At the next step, histoscanning was performed with construction of maps of the prostate, which was used when performing a biopsy. The results of histoscanning were compared with data of morphological examination of biopsy material, and with the data of visual and morphological studies of surgical specimens in patients undergoing radical prostatectomy. High diagnostic value of histoscanning was demonstrated, as even in patients of Group 1 sensitivity was 89%, specificity--96%, in patients of Group 2 and 3--96 and 94%, 99 and 97%, respectively. False positive results occurred in 11% of cases, false negative--only in 0.6%.

The authors describe their experience with cardiac valve prostheses in 2446 patients with acquired heart diseases and hospital mortality of 21.7%. The surgical methods and the tactics in concomitant diseases are described. The methods of plastic operations on the tricuspid valve are presented. Late results of prostheses of the cardiac valve were studied in 1594 patients. Good late results were recorded in 65.9% of cases, satisfactory in 17.1%, unsatisfactory in 4.3%. The late mortality was 12.7%. Analysis of the late complications is given and the ways for increasing the efficacy of cardiac valve prostheses are suggested.

The late results of 1,205 operations for mitral commissurotomy were studied. The investigation embraced 88.3% of patients discharged from the clinic after surgery. The follow-up period was 8 years and 9 months on average. The late postoperative result was good in 47%, satisfactory in 26.4%, and poor in 16.4% of patients. Late mortality was 10.2%; 75% of patients were alive 15 years after the operation. The effect of the stage of the disease, the volume of the reverse blood flow on the mitral valve and the presence and extent of its calcification on the late results of the operation was studied. Longterm follow-up testifies to the high efficacy of closed mitral commissurotomy.
